Instructor Candidate Application
This application is required to be filled out by anyone wishing to become an AHA Instructor (HeartSaver, BLS, ACLS or PALS) before attending an instructor course. |
Instructor Monitoring Form
This form is used initially when an instructor is completing their instructor course and being monitored teaching their first class. It's also used every 2 years to monitor an instructor again as part of the renewal process for the instructor certification. |
Instructor Records Transfer Request
When an instructor wants to align with a different AHA Training Center, they will need to fill out this form. Afterwards, the new Training Center Coordinator (TCC) will sign it accepting the instructor, and the form will then be sent to the old TCC who has 30 days to transfer all of the instructor's records. |
Instructor Renewal Checklist
As part of the instructor certification renewal process which occurs every 2 years, this form must be filled and signed the instructor's Training Center Coordinator or Training Center Faculty performing the renewal. It records the instructor's current contact information, previous teaching history, and has a checklist of the other required steps for renewal. |

The Instructor Candidate Workbook is used during the BLS Instructor course. It contains important information about the AHA, standards, and has frequently asked questions in the back. |
Heartsaver Instructor Candidate Workbook

How to Claim AHA e-Card

The American Heart Association Instructor Network is the website used by the AHA Instructor to update personal information, list AHA courses, download forms, read recent AHA memos as well as the Program Administration Manual (PAM). |
Feedback Devices
By January 31, 2019 the AHA will require the use of an instrumented directive feedback device or manikin in all AHA courses that teach the skills of CPR. It must provide audio or visual feedback on the rate and depth of compressions during CPR Training. This requirement will impact BLS, ACLS and Heartsaver courses. |
AHA Instructor Liability
Insurance Requirement The AHA requires that parties with whom AHA enters into agreements, whether vendors, sponsors or other types of contracts, provide proof of insurance coverage. This is proof that there are funds available to cover any liability that this other party incurs. |
